Car is coming up for 3 yrs old - annual service @ fixed price of £199 seemed attractive.
I also saw reference on the Nissan site to extended warranty - £369 - £499 / year and for £369 the claim limit for any repair is a paltry £500. Certainly @ £499 the cover is more extensive. The cover is not Nissan but Nissan badged from a 3rd party.
Contrast my son's Lexus cover on extended warranty - no real limit (car's value) for any one repair and it costs him £210 /yr (3rd party supplier not Lexus itself)
I think Nissan should be offering better if it is putting it's name to a product!

Most manufacturer endorsed policies limit claims to purchase price. paying £369 to insure against a maximum of £500 claim is monumental lunacy.
Buy a Subaru ;-)

If they've done their sums & come up with a premium & intervention structure that the OP describes, you must assume it's for a reason. The OP has the option though of several, if not a plethora, of alternative 3rd party warranty suppliers. It's like asking why one pair of shoes is more (less) expensive than another & taking umbrage at the fact.
