There was speculation in the immediate aftermath about cross wiring and it's even possible an example was found.

As ever there were several issues that lined up the holes in the cheese including an attempt to run through the issues which was halted by a radio call from ATC.

Nope. The problem was that on previous variants of the 737 the crew had flown, bleed air for the aircon was drawn from the right engine.  The 400 variant didnt.   When smoke entered the cockpit, they misdiagnosed the wrong engine.  </description>

Yes, watched it all.  The root problem was that while the crew knew an engine was dead or on fire, they couldn't tell which one (it was a 2-engine plane).  Since Kegworth there are now devices near the tail to give pilots this info.  I would have expected a more direct feedback from the engine itself !

Because the plane was gliding, the tail hit first and broke off before the front end hit the embankment.  I think about 45 died out of over 100, and luckily there was no fire.</description>

This caused the "good engine" to close down.

The original connectors behind the cockpit switch panel were identical.  These were replaced on all models with right and left hand connectors!</description>

That's what's so sad about it. The photos at the time showed it a few hundred feet from safety. 

I think the BA Boeing 777 crash at Heathrow in 2008 shows that it could have been more survivable. It would have been carnage if it didn't make the airport and came down over a busy London suburb.

There was an evident engine failure as they passed about 28,000 feet climbing out of LHR. After considering the situation they shut down the 'failed' engine. As they were descending into EMA they didn't need power until they were on final approach and when they advanced the power lever the engine didn't respond with the outcome we saw. 

If they'd been a few feet higher they'd have cleared the M1 and, if not making the runway they'd have pancaked on reasonably level ground outside the A/D boundary.

On the south side of the M1 under that very flight path, a major uk bank had its full back up/ switch over / recovery data centre. The BM flight missed its roof by a few tens of feet, as described by several of my ex colleagues on site at the time.

Our sales team and account manager was on the door selling them a relocate/new data centre build the very next morning.</description>
