No more difficult than if he'd walked out for another woman. His family did know and are supporting him so they must have known for some time and accepted it. It is quite probable that his relationship with his wife had morphed into a brother/sister type relationship some time ago.
I would be surprised if coming out was their choice at least right now. More than likely one of the tabloids were looking for fresh victims whose lives they could destroy (Hopefully to suicide) and as Harry & Meg had run their course for the moment at least the Schofield's number came up.
